2022 City of Lincoln COVID-19 Business Recovery Grant Program Submissions

Please submit the following documents to be considered:

1. Completed Grant application

2. Copy of Business License

3. 2019 tax return (Schedule C page) or the corresponding year business opened (business must have opened for business nop later than June 30, 2021)

4. Completed W-9
